All returns must be accompanied by a printed version of this form. Please be as detailed as possible in this form as we will use this information to process your return. Any part returned without a detailed form or a return authorization number are subject to delay. Any parts being returned for warranty will require a proof of purchase. If you do not have the ability to print you may call LSR and a representative can give you a return authorization number. Any parts could be subject to a restock fee.
